SPORTS Health ScreeningS

SVUSD SPORTS Health Screenings

All students participating in Marching Band and Color Guard must have a Health Screening Form completed by a medical professional and turned in prior to the first day of Band Camp (date TBA). We have partnered with Saddleback Family Urgent Care to help students get their screenings completed. Saddleback Family Urgent Care has agreed to donate 50% of the proceeds from screenings back to the program.

It is also fine to arrange to have a screening completed through your own family physician. Just be sure to have your physician complete the necessary forms, linked at the bottom of this page.

How to Sign Up

1. Arrange and appointment date and time for your student using this link here. Fill out your student's name on the form, not their parent's name.
2. Pay the $40 fee and donation on the Web Store. Be sure to include your student's name in the comments section when you make your PayPal donation. Make a note of your PayPal confirmation number, as you will need it for the next step.
​3. Phone Saddleback Family Urgent Care in Mission Viejo to book your appointment. Please see down below for their phone number. They will ask for your PayPal confirmation number from the step above.
​4. Fill-out and bring a hard-copy of the SVUSD Health/Sports Screening Forms, available at the links below. Three forms are required.
​5. Attend your screening appointment, and have your child's examining physician complete the SVUSD Sports Screening Form and return it to you after the exam. This is the form you will have your student turn-in to Mr. Miller. A parent or guardian must attend the appointment with each student.

​Please note: Saddleback Urgent Care will do their best to see your student at their scheduled time. However, there may be some delays if they have patients who need emergency care treatment. Please remember that you are representing our school. Your patience is appreciated.
